can you add documents to a task/action, this would be very useful if an incident record is closed

You have 2 options here i'd say..
1. You could reopen that incident, attach the document properly as necessary, then close it off again. This article here shows you how to do that: https://inphase.zendesk.com/hc/en-us/articles/10148307666076-Reopen-a-Closed-Record-in-Question-Manager
2. You can add attachments to tasks, and you'll find the section to do it at the bottom of the create task dialogue box, like my screenshot below:
